    I work for the State. I'd like to pay by check rather than cc. How do I do that?

      Just print a copy of the invoice. If you plan to pay personally, send a copy of the invoice and your check to MPHA, P.O. Box 14709 Minneapolis, MN 55414. If the State is paying your conference fee you will need to submit the invoice for payment using whatever process your department requires.
